Although Dean wanted to tell him the truth, and go with him, he loves Sam too much to ruin his life. Having a baby is going to change everything. So he smiles and says he'll be there soon. Instead he runs, settling down in a little town in the middle of nowhere, to build himself a new life. He’s happy enough, working two jobs which indulge his love of pie and new found love of reading, and if he can't quite leave Sam behind yet, that's ok.

Meanwhile Sam is waiting for Dean, waiting for them to start their new life together. Maybe Dean should have asked Sam what he thought?
